Customer Requirements Agreement Form


In light of recent internal reviews, we have come up with a few directives to improve our service standards. One of the outcomes of the reviews is the creation of the document: Customer Requirements Agreement. The main purpose of this document is to help us better record and track your calibration needs. The completion of this agreement benefits you because a better understanding of your requirements will in turn translate to leaner work processes that help to improve efficiency.



Moreover, it is specifically stipulated in ISO/ IEC 17025 : 2005 under Clause 4.7 (Service to the customer) that the laboratory shall cooperate with the customer in clarifying the customer’s requests. Hence, your completion of this agreement is crucial in helping us fulfil this condition.


This agreement form takes approximately less than 15 minutes to complete. We will very much appreciate it if you can complete and return this form to us before we embark on calibrating your equipment. Please help to make sure that the person in-charge of your calibration program is notified of the requirements requested and is preferably the representative to complete the agreement form.
